PHYSICAL EXAMINATION

  • Students registering for Athletics must have a current physical examination form submitted for review before the student is authorized to participate in practice or tryouts. The Physical Examination shall not be taken earlier than May 1 preceding the school year for which it is applicable. ATHLETICS FORMS

    All middle school Athletic Packets are online!  Do Not fill out a paper Athletic Packet!  Schedule your doctor’s physical, have them fill out the attached 2 medical provider pages, save the signed pages (page 3 & 4) and upload.

    DO NOT send paper pages from the Athletic packet to the office!!  ALL athletic registration must be done online by parents/guardians and physical examination forms must be uploaded.
