PHYSICAL EXAMINATION

Students registering for Athletics must have a current physical examination form submitted for review before the student is authorized to participate in practice or tryouts. The Physical Examination shall not be taken earlier than May 1 preceding the school year for which it is applicable. Click here for instructions on submitting a completed physical exam.
ATHLETICS FORMS

Athletics forms must be completed and submitted online before a student will be allowed to practice and participate in any sport, cheerleading or dance team activity. Physicals are required by KSHSAA annually and shall not be taken earlier than May 1 preceding the school year for which it is applicable. Please visit https://bluevalleysd-ar.rschooltoday.com/home to submit your student's physical. No paper copies will be accepted in the front office.
